Hamilton Continuing Care Center is a 180 bed facility that includes a 28 bed dementia unit and a 32 bed Sub-Acute unit. We are located in Hamilton, New Jersey. This facility offers a variety of care options in a country setting.

Hamilton Continuing Care Center is located in suburban Hamilton Township, a short drive from Mercer County’s acute care hospital settings. Our campus is across from Mercer County College and convenient to a number of restaurants, shopping centers, historic sights, and activities for the whole family to enjoy.

Subacute Rehab Services
Our Subacute Rehabilitation Services address diverse conditions resulting from debilitating illnesses and injuries that may be a result of:
Neurological conditions, such as stroke, Multiple Sclerosis, and Parkinson's Disease
Orthopedic injuries and fractures
Joint replacements
Cancer-related conditions
Amputations
Trauma resulting from accident or illness
Muscular/skeletal problems
Amputations/prosthetics
Chronic pain/edema
Wound care/decubitus ulcer
Cardiopulmonary dysfunction
Physical Therapy/Whirlpool Therapy
Physiatrist Services
We provide the following therapies under the supervision of an onsite Physiatrist (a physician specializing in rehabilitation medicine):

The Physiatrist supervises each patient's therapy program and participates in the care planning and discharge planning process.
