Everyone residing at The Country Home has health challenges, the primary reason they can no longer live safely at home alone or with family. Most have progressing memory loss related to dementia/Alzheimer's disease. Some have had dangerous experiences with wandering or other life-threatening behavior. They often have a variety of physical illnesses as well, such as diabetes, arthritis, vision or mobility loss, incontinence, coronary or pulmonary disease, etc. The role of the RN is to keep your loved one as healthy as possible, for as long as possible, despite these pre-existing medical conditions!

Our Registered Nurse, who has about 15 years of nursing experience, most of it in a rehabilitation setting serving the elderly, is involved in all aspects of prevention, assessment and treatment as necessary, in conjunction with the resident's physicians and other health professionals. She works at the Country Home at least three days a week but is on call 24/7.
PRE-ADMISSION WORK OF THE RN OR ADMISSIONS STAFF:

* Assesses memory and cognitive impairment
* Assesses or obtains existing records on all other mental and physical conditions, in consultation with the individual's physicians, hospital, nursing home, family, etc.
* Answers questions of family members
* Obtains TB and MRSA test results, requests orders for flu and pneumonia vaccinations if appropriate
* Determines care level needed and appropriateness for The Country Home
* Prepares the health portion of admission paperwork along with standard MDS medical chart
* Obtains all medications the physician has ordered, so they can be administered by Country Home staff as soon as resident arrives. Also obtains oxygen, equipment or anything else the physician has ordered
* Delegates aspects of nursing care by reviewing medical chart and related information with the Manager of Resident Care so all staff know the medical, dietary, social, activity needs of the new resident
